Shoulder pain in active adults is rarely simple. A physiotherapy assessment differentiates rotator cuff issues, impingement, and referred pain from the neck to guide the right treatment.
Symptoms we commonly see
- Pain on reaching overhead or behind the back, particularly when putting on a jacket or reaching a high shelf.
- A painful arc - the shoulder hurts in a specific range of the lift but is less painful at the bottom and top.
- Night pain, especially when lying on the affected shoulder.
- Weakness when pushing overhead, throwing, or performing pressing movements in the gym.
- Clicking, catching, or a sense of instability with certain movements.
What drives shoulder pain in active adults
The shoulder is the most mobile joint in the body and relies heavily on dynamic muscular control. Most pain has a mechanical rather than structural origin.
- Rotator cuff overload and partial tears. The rotator cuff muscles fatigue under sustained overhead loading, repetitive press volume, and racquet sport asymmetry. This is accelerated in Doha's gym culture where pressing volume often exceeds pulling and shoulder stability work.
- Subacromial impingement. Compression of soft tissue in the subacromial space is common when thoracic mobility is limited and the scapula moves poorly. Many desk workers present with this pattern.
- Cervical referral. Pain perceived in the shoulder or upper arm can originate from the lower cervical spine. Treating the shoulder alone fails in these cases, which is why accurate assessment is essential.
How a physio-led session helps
We begin with a comprehensive shoulder assessment: range of motion, specific impingement tests, rotator cuff strength testing, and a cervical screen to rule out referred pain. This takes the guesswork out and gives us a clear target.
Hands-on work focuses on the structures under load. This may include manual therapy to the glenohumeral joint and surrounding capsule, soft tissue release of the rotator cuff and posterior shoulder, and thoracic mobility work to improve scapular mechanics. We combine this with specific shoulder loading exercises appropriate to your stage of recovery.
We are realistic about timelines. Acute impingement presentations often respond in 4-6 sessions. Rotator cuff pathology with significant strength deficit may need 8-12 sessions plus a structured home programme. We give you an honest plan after the first assessment.
When to escalate beyond physio
Some shoulder presentations require imaging or specialist referral before or alongside physiotherapy.
- Significant trauma to the shoulder, such as a fall on an outstretched arm or a dislocation.
- Rapid onset of severe weakness suggesting a complete rotator cuff tear.
- A hard lump, unexplained swelling, or skin changes around the shoulder joint.
- Shoulder pain in a person over 50 with a significant recent activity change or without a clear mechanical explanation.
- No improvement after 6 sessions of appropriate physiotherapy.

Common questions
Do I need a scan before physiotherapy?
In most cases, no. Clinical assessment gives us enough information to begin treatment. If there is no improvement after a reasonable course, or if the clinical picture suggests a complete tear, we will recommend imaging.
Can I keep training my upper body?
Usually yes, with modifications. We identify which movements load the painful structures and substitute alternatives until you recover enough to reintroduce them progressively.
Will my shoulder ever be the same again?
For the vast majority of non-surgical shoulder conditions, full or near-full function is a realistic outcome with the right rehabilitation. The key is addressing both the symptom and the movement patterns that created the problem.
My shoulder has been sore for years. Is it too late to treat it?
