Runner's Knee in Doha - Physio Assessment at MVP
Runner's knee covers several distinct diagnoses. A physiotherapy assessment identifies which structure is involved so that treatment targets the right cause, not just the symptom.
Symptoms we commonly see
- Pain at the front of the knee (under or around the kneecap) that comes on during or after running.
- Pain on the outer side of the knee that appears after a consistent distance and forces you to stop.
- An ache below the kneecap that is worse in the morning or after sitting, easing once warmed up but returning after long runs.
- Swelling or puffiness around the kneecap or below it.
- A feeling that the knee wants to buckle when descending stairs after a run.
The most common runner's knee presentations
Runner's knee is a colloquial term that covers at least three distinct conditions with different causes and treatments.
- Patellofemoral pain syndrome. Abnormal loading of the cartilage behind the kneecap from hip weakness, poor hip drop control, or overpronation. Very common in runners who have recently increased their mileage or changed their footwear.
- IT band syndrome. Friction of the iliotibial band over the lateral femoral epicondyle, classic at the 3-5km mark in a run. Linked to hip abductor weakness, poor running cadence, and sudden increases in weekly volume.
- Patellar tendinopathy. Repetitive tensile load on the patellar tendon from high-intensity running, hill work, and speed sessions. Common in runners who also do jumping sports or heavy gym work.
How a physio-led session helps
We begin with a full running-focused assessment: range of motion at hip, knee, and ankle, strength testing of the hip abductors and glutes, and - if needed - a running gait analysis on the treadmill at Virgin Active. Identifying the specific diagnosis is the foundation of everything else.
Treatment is matched to the diagnosis. Patellofemoral pain responds to hip strengthening, patellar taping, and technique modification. IT band syndrome requires a combination of hip work, foam rolling guidance, load management, and cadence coaching. Tendinopathy needs progressive tendon loading, not rest. Each plan is different.
We give every runner a realistic return-to-running timeline with a structured load progression rather than a binary stop-or-go. Most presentations allow some running to continue throughout treatment, with load and intensity appropriately adjusted.
When to escalate beyond physio
Most runner's knee is manageable with physiotherapy, but some presentations need further investigation.
- Significant swelling inside the knee joint that came on without a specific incident.
- Locking or catching of the knee joint, suggesting a meniscal or loose body issue.
- Pain that is constant and present at rest, not just with running.
- Knee pain in a runner under 16 years old, which may indicate an apophyseal or growth plate issue.
- No improvement after 8 weeks of appropriate physiotherapy and load management.

Common questions
Do I have to stop running completely?
In most cases, no. We adjust your running load rather than eliminating it. Complete rest often delays recovery and deconditions the tissues we need to load progressively. You will run less, not stop.
I was told to foam roll and stretch my IT band. Why is it not getting better?
The IT band itself does not significantly lengthen with stretching. The source of IT band syndrome is usually hip weakness and running mechanics, not a tight band. Targeting the right structures produces results.
How long until I can run a race?
It depends on which structure is involved and how long it has been symptomatic. A mild patellofemoral presentation in a runner with good baseline fitness may return to racing in 4-6 weeks. Established tendinopathy may need 3-4 months of structured loading first.
Should I change my running shoes?
Footwear is one input, not the whole answer. We assess your mechanics and foot pronation as part of the evaluation and will advise on footwear if it is contributing to your presentation.
