Why structured rehab after surgery matters

Post-surgical rehabilitation is not a passive process. The quality of recovery depends directly on what you do in the weeks and months following surgery.

  • Neuromuscular inhibition. After joint surgery, the nervous system inhibits muscle activation around the operated area as a protective response. Restoring this activation is not automatic - it requires specific loading and motor control work.
  • Tissue remodelling windows. Tendons, ligaments, and joint capsule tissue heal through defined biological stages. Loading at the right time and in the right way accelerates this process; loading too early or too aggressively can compromise the repair.
  • Return-to-sport criteria. Returning to running, sport, or heavy gym work based on time alone significantly increases re-injury risk. Criteria-based progression - passing strength and movement benchmarks - is the safe standard.

How a physio-led session helps

We begin with a full post-surgical assessment: review of the surgical report and any imaging, measurement of range of motion and swelling, strength testing where appropriate, and a clear conversation about your goals - whether that is returning to a sport, a job, or simply daily life without restriction.

Treatment is structured in phases aligned with your healing timeline. Early phase work focuses on swelling control, range restoration, and progressive neuromuscular activation. Intermediate phase introduces functional strengthening and movement pattern retraining. Late phase - return to sport or full function - is criteria-based, not time-based.

We liaise directly with your surgeon where needed. If a finding at reassessment warrants review, we communicate clearly. Our goal is not just symptom resolution but a full return to the activities you value, with the confidence that the structure is ready.

When to escalate beyond physio

Some post-surgical signs require prompt communication with your surgical team.

  • Signs of infection at the surgical site: increasing redness, warmth, discharge, or fever.
  • Sudden significant increase in swelling or pain without a clear loading reason.
  • Numbness, tingling, or weakness that is new or worsening rather than improving.
  • A pop, snap, or giving-way that suggests a re-rupture or failure of the repair.
  • No objective improvement in range of motion or strength over 3-4 weeks of consistent physiotherapy.

Common questions

When should I start physiotherapy after surgery?

It depends on the procedure. Many protocols begin within days of surgery for range of motion work. Others have a protected period before loading. We work within the protocol your surgeon specifies and communicate with them if needed.

My surgeon said I can return to sport at 6 months. Is that accurate?

Time since surgery is one factor. What matters more is whether you have passed the strength and movement quality benchmarks appropriate for your sport. At 6 months, many patients have not yet met the recommended strength ratios. We test this objectively.
